1, The Flaintiff is providian National Bank f/k/a First
Deposit National Credit Card Bank, a national bank with a place
of business located at 53 Regional Drive, concord, NH.
2. The Defendant is Timothy S. Rake, an individual with a
place of residence located at 6035 Wertzville Road, Enola, PA
17025.
3. At the written request of the Defendant, Monogram Bank
USA isyued to Defendant a MASTERCARD credit card, account number
5409812620212062 and at all times relevant hereto, Defendant was
the holder of said card issued through Monogram Bank USA's credit
facilities.
4. Defendant, upon acceptance of Plaintiff's credit card,
agreed to be bound by the terms and conditions of Plaintiff's
revolving credit plan agreement.
