1. The Petitioner, Lisa Mullet, is the Plaintiff in the
above reference matter and an adult individual residing at 1824
Heishman Gardens, Carliale, Pennsylvania 17013.
2. Respondent in thia matter is David Lee Mullet who
currently resides at 17928 Garden Lane, No. 31, Hagertown, Maryland
21740.
3. partiea were married on June 15, 1984, in Hagerstown,
Maryland.
4. A Divorce Complaint was filed in the Cumberland County
Court of Common Pleas on June 29, 1995.
5. Service upon the Defendant was attempted by Certified
Mail to the address as set forth in paragraph 2 above. Said
notification was returned unclaimed after three attempts on July 3,
1995, July 8, 1995, and July 18, 1995.
6. Service was again attempted by regular mail with a
acoeptance of service form to be returned by Defendant. Said
correspondence was Bent on August 23, 1995, and never returned.
7. Respondent ia able to completely, competently able to read
and write the English language to the best knowledge, information,
and belief of the Petitioner.
