ORDER OF COURT
AND NOW, this ,11. day of July, 1995, upon consideration of
the Petition For Change of Name, it is ORDERED that a hearing on
Petitioner's request be held on Tuesday, August 29, 1995, at 3130
p.m., in Courtroom No.5, Cumberland County Courthouse, Carlisle,
Pennsylvania.
IT IS FURTHER ORDERED that the notice of the filing of said
Petition and the aforesaid date, time and place of the hearing be
published once in the Cumberland Law Journal and a newspaper of
general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioner shall comply with all other
requirements of C.C.R.P. 5, and the Act of December 16, 1982, P.L.
1309, 56, and 54 Pa. C.S. 55701 et seq. (1994 Supp.). Notice to
the natural father shall be by personal service or by certified
mail, return receipt requested, restricted delivery. Notice by
certified mail shall not be considered adequate unless the natural
father has personally signed the receipt card.

PETITION FOR CHANGE OF NAME
TO THE HONORABLE, THE JUDGES OF THE SAID COURT
The petitioner, Michelle Theresa DeMerice, by and through her
undersigned counsel, requests this Court to order a change of name for
her minor son, ERIC BRIAN CARROLL, pursuant to 54 Pa. C.S.A. ~701, et
seq" and in support thereof represents as follows:
1. The petitioner, Michelle Theresa DeMerice, is an adult
individual residing at 540 Harding Street, New Cumberland, Cumberland
County, Pennsylvania.
2. The petitioner is the natural mother of Eric Brian Carroll, a
minor, date of birth May 18, 1981. The petitioner and Eric Brian
Carroll have resided at the following locations during the five years
preceding the filing of this petition: 341 Hummel Avenue, Lemoyne,
Cumberland County, Pennsylvania from August, 1987 to September, 1994;
and 540 Harding Street, New Cumberland, Cumberland County, Pennsylva-
nia from September, 1994 to the present date,
3. The petitioner and MICHAEL WARREN DeMERICE, an adult indi-
vidual, were married January 18, 1992, at the Grace United Methodist
Church, Lemoyne, Cumberland County, Pennsylvania.

4. The petitioner and Michael Warren DeMerice have one child,
CHELSEA NICOLE DeMERICE, born October 7, 1989. The petitioner,
Michael Warren DeMerice, Eric Brian Carroll and Chelsea Nicole
DeMerice all reside at 540 Harding Street, New cumberland, Cumberland
County, Pennsylvania.
5. Carroll, being the last name currently used by Eric Brian
Carroll, is the maiden surname of the petitioner, Michelle Theresa
DeMerice.
6. Eric Brian Carroll wishes that his last name be the same as
his step-father, half-sister, and natural mother.
7. The natural father of Eric Brian Carroll is Albert L, Comer,
an adult individual residing at
Attached to this petition is the consent of Albert L. Comer to the
change of name requested in this petition,
8, The petitioner and Albert L. Comer never married and Eric
Brian Carroll was born out of wedlock.
9. Attached to this petition is the consent of Michael Warren
DeMerice, the spouse of petitioner to the change of name requested
herein.
10. There are no judgments or decrees of like character of
record against Eric Brian Carroll.
