The Petition of MARYANN MURPHY, by and tl"ough her attorney,
Joanne Harrison Clough, Esquire, on behalf of MICHABL ALAN BUHRIG,
the minor child, respectfully represents as follows:
1. Petitioner, MARYANN MURPHY, is an adult individual who
resides at 206 Cherokee Drive, Mechanicsburg, Cumberland County,
Pennsylvania, 17055,
?. Petitioner is the mother of MICHABL ALAN BUHRIG.
3. The father of the child is ROBBRT L. BUHRIG, Sr., who
currently resides at 3928 Thawley Court, Tucker, DeKalb County,
Georgia, 30084.
4. Petitioner and the father were divorced in 1986.
5. Petitioner is unmarried.
6. The fathor is married to catherine Lynch, Esquire,
7. HICHABL ALAN nUHRIG is thirteen (13) years old, having
been born in Tampa, Flor Ida on August 28, 1981. ^ copy of the
certificate of birth is attached hereto, made a part hereof and
marked as Exhibit "A".
8. HICIlABL ALAN BUHRIO res ides with his mother and brother,
THOHAS JOSEPH MURPHY, at 206 Cherokee Drive, Mechanicsburg,
Cumberland County, Pennsylvania.
9, Petitioner resumed her original name, MURPHY, at the time
of the divorce in 1986.
10. The minor child's brother, THOHAB JOSEPH HURPHY, had his
name changed to that of Petitioner by the Court of Common Pleas of
Cumberland County, Pennsylvania in 1991.
11. The minor child has always resided with Petitioner, who
was awarded primary phyeical and legal custody at the time of the
divorce.
12. The minor child has not seen hie father, ROBERT LANCE
BUHRIO, Sr., since 1986, although the father was granted visitation
of the child.
13. The minor child has had no significant contact with the
f ather since the time of the divorce, other than birthday and
Christmas cards.
14. The minor child has had no significant contact with his
paternal grandparents since 1986, and no contact whatsoever for tho
past several years. The paternal grandfather is now deceased.
15. The minor child has consistently expressed his desire to
have his name changed to that of Petitioner bocause of his close
relationship with Petitioner and his maternal grandparentK.
16. The minor child will be entering high school in the fall
of 1995, and desires to have his name changed to "MURPHY" prior to
that time.
17, The minor child's siblings are supportive of his desire
to have his name changed to "MURPHY".
18, Petitioner believes that it is in the beet interests of
the minor child that his name be changed to "MURPHY".
19. The father has indicated to Petitioner that he will not
object to the name change.
WIIBRBFORB, Petitioner prays this Honorable Court to enter a
decree changing the minor child's name from HICHABL ALAN BUHRIG to
MICHABL ALAN MURPHY.

having been born on January 15, 1947. I am currently married. The
minor child in this matter, MICHAEL ALAN BUHRIG, is my adopted son.
MICHAEL was born on August 28, 1981, and was adopted by MARYANN
MURPHY and myself. I hereby voluntarily and unconditionally consent
to the change of name of MICHAEL ALAN BUHRIG to MICHAEL ALAN
MURPHY.
I understand that by signing this Consent I indicate my intent
to permit my son to have his name legally and permanently changed.
I understand that I may revoke this Consent to permit my son
to have his name legally and permanently changed by placing the
revocation in writing and serving it upon Joanne Clough, Esquire,
2201 North Second Street, Harrisburg, Pennsylvania, prior to the
entry of a decree changing my son's name.
I understand I may not revoke this Consent after a Court has
onterod a decree changing my son's name.
I waive notice of, and ask to be excused from appearing at allY
hearing to be held at any time in the future in connection with the
name change of my son because I do not wish to attend any such
hearing.
