re/liding at the addre/l/l set forth in the caption hereof.
3. On or about August 12, 1983, the Defendant signed a
Revolving Loan Agreemf.lnt (hereinafter referred to as "Agreement")
with the Plaintiff, a lending institution.
4. The aubject Agreement is at~ached hereto as Exhibit
A and incorporated by reference herein.
5. The agreement whereby Plaintiff and Pefendant entered
into provides that the Defendant was to be given a maximum credit
limit in the amount of $3,500,00. Prom time to time, that credit
limit has been increased,
6. The agreement whereby Plaintiff and Defendant entered
into provides that the Defendant could obtain funds directly
through the Plaintiff or through the use of special checks until
the total unpaid balance including finance charges, and insurance
oharges, if any, reached the maximum credit limit,
7. The agreement whereby Plaintiff and Defendant entered
into provides that the Defendant was to pay to the Plaintiff the
account indebtedness in consecut i va monthly installments of at
lea/lt the amount computed according to a

Minimum Payment Sohedule, said acoounl;. indebtednelilfl inoludes a)
the amount borrowed, b) finance oharge and other ohargee provided
in the agreement, c) insuranoe charges, if any, d) colleotion costs
permitted by law including reasonablo attorney' a fees, and e)
amounts in excees of the maximum credit limit. said paymenta are
to be made each and every month until the account indebtednesa ia
paid in full.
B. The Defendant ia delinquent in fulfilling the

thie account and has faileti to keep the paymente up-to-date with
the last payment having been made on or about May 10, 1995,
