That Your Petitioner is STEPHANIE LYNN MECK, an adult individual currently residing at 516 Carol Street, New Cumberland, County of Cumberland, Pennsylvania. 2. That Your Petitioner desires to change her name to NICOLE LYNN MECK. Your Petitioner has been known generally to her friends and acquaintances as "Nicki" all of her life. Additionally, Your Petitioner's father and brother are both named "Stephen" and the name similarities have caused confusion. 3. That Your Petitioner has resided at the following addresses during the past five years: from July, 1990, until October, 1991, at 125 Sugar Maple Lane, Etters, County of York, Pennsylvania; from October, 1991, until April, 1993, at 2217 South Third Street, Steel ton, County of Dauphin, Pennsylvania; and from April, 1993, until the present at 516 Carol Street, New Cumberland, County of Cumberland, Pennsylvania. WHEREFORE, Your Petitioner respectfully requests that Your Honorable Court pursuant to the Act of April 18, 1923, P.L. 75, Section 2, as amended, 54 P.S.
