AFFIDAVIT AND ORDER OF COURT
On this 4th day of June 2002, the below-signed individual
personally appeared before me, was sworn under oath and
presented the following facts:
That on or about October 9, 2001, he purchased a
1990 Firebird (Pontiac) bearing VIN #1G2FS23E2LL214843
from Lemoyne Auto Sales in New Cumberland, Pennsylvania;
That at that time he paid the purchase price, including
Tag/Transfer Fee ($68.50) and Taxes ($149.70) [Total
Paid was $2,778.20];
- That at this time a Temporary Registration (pink slip)
was issued by the Dealer [See Copy as Attached];
That it was his strong belief that Lemoyne Auto Sales
would follow through by sending in the necessary paper-
work to PennDOT;
That it was later learned that PennDOT had never received
any of the paperwork, nor the monies that were related
to this transaction;
That additionally, it was learned that the Cumberland
County District Attorney's Office was investigating
many issues surrounding the operation of Lemoyne Auto
Sales;
That in effort to formally, prpoerly and legally register
this vehicle, I respectfully request that the Commonwealth
of Pennsylvania's Department of Transportation issue a
Title and Registration in my name, as reflected on the
attached document.
