through his attorneys, MARTSON DEARDORFF WILLIAMS & OTTO, files the following Petition
for Change of Name:
1. Petitioner, Timothy Lee Wells, is a single adult individual residing at 1004 Northfield
Drive, Carlisle, Cumberland County, Pennsylvania and has resided at this address throughout the
five-year period immediately prior to the filing of the Petition for Change of Name.
2. Petitioner desires to have his name changed from Timothy Lee Wells to TIMOTHY
LEE CURTIS.
3.
Petitioner desires to have his name changed because of a personal choice to take his
mother's maiden name.
4. Petitioner is a citizen of the United States of America, having been bom in Shaker
Heights, Ohio on January 1, 1978.
5. Petitioner is a single adult individual who is the age of twenty-four (24) years.
6. If this Petition is approved, Petitioner intends to use the name TIMOTHY LEE
CURTIS consistently, non-fraudently, and exclusively.
7. It is not the purpose of this Petition to avoid any taxes or any other debts.
WHEREFORE, Petitioner requests this Honorable Court to enter an Order of Court changing
his name from Timothy Lee Wells to TIMOTHY LEE CURTIS.

Petition for Change of Name, a hearing is scheduled for Monday, November 25, 2002, at
11:00 a.m., in Courtroom No. 1, Cumberland County Courthouse, Carlisle, Pennsylvania.
IT IS FURTHER ORDERED that notice of the filing of said Petition and the
aforesaid date, time and place of the hearing be published once in the Cumberland Law
Journal and a newspaper of general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioner shall comply with all other requirements of C.C.R.P. 5,
and the Act of December 16, 1982, P.L. 1309, §6 (see 54 Pa. C.S. §701 [Historical and
Statutory Notes] [Main Volume]), and 54 Pa. C.S. §§701 et seq.
