CIVIL ACTION
1. The Plaintiff is BANK ONE BANK CARD DIV. (hereinafter,
Plaintiff) a corporation.
2. The Defendant(s) is/are ROBERT H. SHANK and (hereinafter,
Defendant(s)), and individual(s) with a place of residence located
at 4902 CARLISLE, STE 282,MECANICSBURG, PA 17055.
3. The Defendant was issued a CREDIT CARD from Plaintiff in
accordance with the application. A true and correct copy of the
same is attached hereto, made a part hereof and marked Exhibit
itA".
4. The Defendant accepted and used the CREDIT CARD in accordance
with the rules and regulations of said credit card. A true and
correct copy of same is attached hereto, made a part hereof and
marked Exhibit "B".
5. As of 07-24-95, the Defendant's outstanding balance was
$2,443.42. A true and correct copy of a statement of account in
connection with said CREDIT CARD is attached hereto, made a part
hereof and marked Exhibit "C".
6. Notwithstanding the frequent demands by Plaintiff for payment
of the amount due, there has been failure and refusal by the
Defendant to pay the same or any part thereof pursuant to Exhibit
fiB" .
7. In accordance with the terms of Exhibit "B", Defendant agreed
to pay Plaintiff a finance charge on all sums due at an annual
percentage rate of 24.00% per annum for the period from 05-05-94.
8. In accordance with the terms of Exhibit "B", Defendant agreed
to pay to Plaintiff a reasonable attorney's fee if the account was
referred to an attorney for collection, and Plaintiff will incur
an attorneys fee in the amount of $488.68.
WHEREFORE, Plaintiff demands Judgment against the Defendant in the
sum of $2,932.10 plus interest from 05-05-94 and costs of this
action.

1. The Plaintiff is BANK ONE BANK CARD DIV. (hereinafter,
Plaintiff) a corporation.
2. The Defendant(s) is/are ROBERT H. SHANK and (hereinafter,
Defendant(s)), and individual{s) with a place of residence located
at 4902 CARLISLE, STE 282,MECANICSBURG, PA 17055.
3. The Defendant was issued a CREDIT CARD from Plaintiff in
accordance with the application. A true and correct copy of the
same is attached hereto, made a part hereof ann" marked Exhibit
"An.
4. The Defendant accepted and used the CREDIT CARD in accordance
with the rules and regulations of said credit card. A true and
correct copy of same is attached hereto, made a part hereof and
marked Exhibit "B".
5. As of 07-24-95, the Defendant's outstanding balance was
$2,443.42. A true and correct copy of a statement of account in
connection with said CREDIT CARD is attached hereto. made a part
hereof and marked Exhibit "C".
