ORDER OF COURT
AND NOW, this "t~ day of October, 1995, upon consideration of
the Petition For Change of Name, it is ORDERED that a hearing on
Petitioner's request be held on Thursday, November 30, 1995, at
3:30 p.m., in Courtroom No.5, Cumberland County Courthouse,
Carlisle, Pennsylvania.
IT IS FURTHER ORDERED that the notice of the filing of said
Petition and the aforesaid date, time and place of the hearing be
published once in the Cumberland Law Journal and a newspaper of
general circulation in the County at least 10 days prior to the
hearing~ in addition, Petitioner shall comply with all other
requirements of C.C.R.P. 5, and the Act of December 16, 1982, P.L.
1309, 56, and 54 Pat C.S. 55701 et seq. (1995 Supp.).

IN THE COURT OF COMMON PLEAS OF

NO. 95- CIVIL TERM
PETITION FOR CHANGE OF NAME
TO THE HONORABLE, THE JUDGES OF SAID COURT:
AND NOW, comes the Petitioner, Gregory Lynn Gates, by and
through his attorneys, Fowler, Addams, Shughart & Rundle, and
represents the following:
1. The Petitioner is Gregory Lynn Gates, who currently
resides at 1790 Cessna street, Mechanicsburg, Cumberland County,
Pennsylvania.
2. The Petitioner has physically resided in Cumberland
county, Pennsylvania continuously since September, 1991.
3. Prior to September, 1991, the Petitioner was on active
military duty as a member of the United States Marine Corp with
his last duty station being Camp Lejune, North Carolina.
4. The Petitioner is a man, but is presently taking steps
to have his sex changed to that of a woman.
5. The Petitioner has dressed exclusively as a woman for a
period of time in excess of three years.
6. The Petitioner is under the care of a physician and is
taking daily doses of estrogen in preparation for a sex change
operation.
7. The Petitioner is presently under the care of a licensed
psychologist in preparation for a sex change operation.
8. The Petitioner has informally used the name "Rebecca"
socially for a period of time in excess of three years.
9. The Petitioner desires to legally change his name from
Gregory Lynn Gates to Rebecca Lynn Gates.
10. There are no judgments or decrees of like chalacter of
record against the petitioner.
11. The Petitioner does not seek to change his name for the
purpose of avoiding payment of taxes or other debts or to defraud
creditors.
12. The Petitioner's sole purpose in changing his name is
in furtherance of his desire and intention to physically change
his sex from male to female.
WHEREFORE, the Petitioner requests this Honorable Court,
pursuant to the Act of December 16, 1982, P.L. 1309, No. 295,
Section 2, 54 Pat C.S. S702, 59 do the following:
a. Fix a hearing on this Petition;
b. Direct that notice be given of this Petition and
hearing; and
C. Enter a Decree changing his name to Rebecca Lynn
Gates.

1. That he is an attorney-at-law admitted to the Bar of the
State of North Carolina, maintaining an office at 327 New Bridge
Street, Jacksonville, Onslow County, North Carolina.
