TO THE HONORABLE, THE JUDGES OF THE SAlD COURT:
AND NOW COMES Rhonda Harlacher, by her attomeys, Mette, Evans &
Woodside, and respectfUlly represents that:
1. Petitioner is the parent of Erica Nicole Harlacher, a minor, who was
born July 3, 1986, and resides at 222 Eutaw Avenue, New Cumberland, PA 17070,
with Petitioner.

policy is maintained with Cigna Group Insurance with a total value of Eighteen
Thousand ($18,000) Dollars,
4. Erica N. Harlacher is a fifty (50%) beneficary under this IIOllcy, and
thus, the minor's interest will be Nine Thousand ($9,000) Dollars, Petitioner
Rhonda Harlacher's deceased husband, Eric Harlacher, was the owner of the
policy.
G. The minor has no other estate or interest in any other estate or trust,
WHEREFORE, petitioner requests that the court authorize her, as natural
guardian, to receive the distribution of less than Twenty-Five Thousand ($25,000)
Dollars from the life insurance proceeds on the life of minor's father, Eric
Harlacher, Deceased, in accordance with 20 Pa, Cons, Stat. t 5101 , without the
appointment of a guardian or the entry of any security,

petition 01 Rhonda Harlacher, It Is hereby ordered and decreed that the proceeds from
the lIIe Insurance be deposited as follows:
1.) Eight Thousand ($8000,00) Dollars 01 the proceeds payable to Erica N,
Harlacher from Clgna Group Insurance shall be deposited In Pennsylvania National
Bank, New Cumberland, In the name 01 Erica N. Harlacher,
2.) One Thousand ($1000.00) Dollars 01 the proceeds payable to Erica N.
Harl8cher from Clgna Group Insurance shall be paid directly to Rhonda Harlacher,
mother and natural guardian 01 Erica N. Harlacher, The One Thousand ($1000.00)
Dollars shall be held eKcluslvely for the benellt 01 Erica N, Harlacher,
3.) The deposit book under paragraph (1) shall be marked: 'Not to be withdrawn
before July 3, 2004, eKcept on Order 01 Court eKcept lor payment of state and federal
Income taKes on the Interest earned by the mlno~s estate or on Order of Court."
4.) The deposit book under paragraph (1), marked as directed, shall be produced
to the Court for Inspection and approval Immediately upon receipt by the Peltltloner.
The Petitioner Is lurther authorized and directed to deposit Interest received from
Income belonging to the minor, II any, In the savings account opened under paragraph
(1), pursuant to this decree,
5.) Pennsylvania National Bank, New Cumberland, may pay any amounts In the
account to Erica N. Harlacher when she attains majority, without lurther order of the
Court,
e.) The Petitioner Is directed to IlIe an Inventory within ninety (90) days 01 the date
of this decree, In accordance with the provisions 0120 Pa, Cons. Stat. 65142 , No
additional assets are to be received by the Petitioner without lurther order 01 this
Court.
