COMPLAINT
COUNT I
1. Plaintiffr or its assignorr is a national banking
associationr with an office located as indicated.
Defendant is an individual residing as indicated.
2. Defendant applied forr and was granted a Master
card/Visa by plaintiff bearing account number
4081704030004148. A copy of the application is attached
as Exhibit "A".
3. Defendant agreed to be bound by the terms of the
account agreement. A copy of which is attached as Exhibit
"Btt,
4. Defendant made use of said creditr and the present
balance owed plaintiff is shown on the last statement
attached as Exhibit "C"r and also shown below, but the
defendant has failed to repay the same as agreed despite
demand.
5. The credit card account agreement and disclosure
statement attached as Exhibit "B" and incorporated herein
by reference also provided for "reasonable" co11eotion
fees. It is averred that the fee set forth below is
reasonable.
6. Because of the foregoingr there is now due the
following sums:

7. Paragraphs 1-6 are incorporated he~ein as if set
forth at length.

5436814030004045. A copy of the application is attached
as Exhibit "0".
8. Defendant made use of said creditr and the present
balance owed plaintiff is shown on the last statement
attached as Exhibit "E"r and also shown here below, but
defendant has failed to repay the same as agreed despite
demand.
