1. Your Petitioner is James W. Whitehill, sui juris, born
Deceaber l2, 1965, and has Pennsylvania Commercial Driver's License
No. 246-28-581.
2. Respondent is the Department of Transportation,
Commonwealth of Pennsylvania.
3. Your Petitioner, was notified by Offioial Notice, dated
October 9, 1995, that his operating privileges would be
disqualified for a period of sixty (60) days effective November 1l,
1995. Attached hereto, marked "Exhibit A" and incorporated herein
by reference is a true and correct photostatic copy of said Notice.
