Ohio 44115, having been first duly sworn, depose and say that
I am over 18 years of age, not a party to this action, that on
December 15, 1995 at 1:00 P.M., I served a Writ of Summons and
a Request for production of Documents, a copy of which is
attached hereto and made a part hereof, upon Towlift Equipment
Company, 1395 Valley Belt Road, Cleveland, Ohio, 44131-1474
by serving David Bongorne, Vice President of Finance.
