AND NOW, come John S. George and Sally A. Myers. by an through Frey & Tiley,
Allomeys lit Lnw, nnd makes the following Petition:
I. Petitioner, John S. George, is an IIdult individulIl, residing lit Carlisle,
Pennsylvnnlll 17013. He is the nlltural fllther of Krystyn N. George, the subject of this Petition.
2. Petitioner, Sally A. Myers. is an IIdult individual, residing at 1138 Newville ROlld,
Cllrllsle. Pennsylvllnlll 17013. She is the natural mother of Krystyn N. George, the subject of
this Petition.
3. The subject of this Petition is Krystyn N. George ("the child"), a minor, born April
19.1986, who resides with her mother at 1138 Newville Road, Carlisle, Pennsylvania 17013.
4. At the time of the birth of the child, Petitioners were husband IInd wife, being
known as John S. George and SlIlIy A. George.
S. Allhetimeofthebirthofthechild, the child took the name Krystyn N. George. A
copy of the birth certificate of the child evidencing this fact is IIlluched hereto and is incorporated
herein as Exhibit "A".
6. Subsequent to the birth of the child, Petitioners divorced, and mother has
remllrried, being now married to Stephen P. Myers. Since the separation of the father and mother,
the child has lived exclusively with the mother, Sally A. Myers.
7. Petitioners now both desire to have the name of the child changed to Krystyn N.
Myers.
8. Petitioners believe it is in the best interest of the child to have her name chllnged to
thllt of Krystyn N. Myers so liS to aid her in feeling a part of a family unit, having the slime nwne
liS her mother IInd siblings.
