b) From April, 1990 until April, 1994 with the I'laintilrnnd the Detimdant

Street. Han'isburg. Dauphin County, Pennsylvanin; and
c) From April. 1994 until the present at 4800 I. Chnrles Rond.

7, The Plaintifr is the til!her of the minor child, lie is unmarried and lives
alone.
8. The Detendant is the mother of the child. She is unmarried and lives with
the child.
9, Plaintitrhas not panicipated ns a party or witness, or in another capacity, in
other litigation concerning the custody of the child in this or another coun.
Plaintitl' has no inliJrlnation of a custody proceeding concerning the child
pending in a coun of this Commonwealth or in another state or commonwealth.
Plaintitr does not know of a person not a party to the proceedings who has
physical custody of the child or claims to have custody or visitation rights with respect to
the child.
10, The best interest and permanent welfare of the child will be served by
granting the relief requested because Plainlitl' is the child's lather and has always been
close wi t h her.
II. Each parent whose parental rights to the child have not been terminated

1.
Mother and Father shall share legal custody.
Mother
shall retain sole discretion for all decisions made regarding
religion and education. However, the parties specifically agree
that Father maintains the right to receive report cards and
participate in parent/teacher conferences and to be kept apprised
of the child's progress in school.
2. Mother shall have primary physical custody of the child.
Father shall have periods of partial physical custody as follows:
A. (1) Durinq February 1996, every saturday from
12100 p.m. to ~IOO p.m.
During March 1996, evqry other Saturday from
10100 a.m. to 8:00 p.m. and every other sunday
from 12100 p.m. to 4:00 p.m.
From April 1996 to June 14, 1996, every other
weekend from 10100 a.m. Saturday to 4:00 p.m.
Sunday.
(2) From June 14, 1996 forward, Father shall have
periods of partial physical custody every
other weekend from Friday at 7:00 p.m. until
sunday at 4:00 p.m.
(3) Every Wednesday from 6:00 p.m. to 8:00 p.m.
B. Holidays
(1) The parties shall alternate the 4th of July,
Memorial Day and Labor Day. This alternating
schedule shall begin with Father having
Memorial Day 1996. custody will continue from
9:00 a.\II. until 5:00 p.m. If this holiday
falls in conjunction with Father's weekend,
