Gutshall, currently residing at 33 Chestnut Street, Newville, Pennsylvania, 17241. She is single. The father of the child is Gary L. "ess, currently residing at 1812 Jumper Street, Cayce, South Carolina, 29033. "e is single. 4. The relationship of Plaintiff to the child is that of natural mother. The Plaintiff currently resides with no other persons. 6. The relationship of Defendant to the children is that of natural father. Plaintiff does not know whether Defendant currently resides with any other persons. 6. Plaintiff has not participated as a party or witness, or in another capacity, in other litigation concerning the custody of the child in this or another court. Plaintiff does not have information of a custody proceeding concerning the child pending in a court of this Commonwealth. Plaintiff is not aware of a person not a party to the proceedings who hus physical custody of the child and claims to have custody or visitation rights with respect to the child. 7. The best interest and permanent welfare of the child will be served by granting the relief request becausel a. Plaintiff Is better ab10 to care for the physical, emotional, educational and medical needs of the child; b, Defendant has not seen the child since birth; furthermore, he has not contacted Plaintiff to make inquiry about hiH Hon since birth; and c. To the best knowledge of Plaintiff, Defendant remains employed as a truck driver and Is away from home on a frequent basis and for long periods of time. 8. Each parl'nt who parental rights to the child have not been terminated and the person who has physical custody of the child has been named as parties to this action.
