Gutshall, currently residing at 33 Chestnut Street, Newville, Pennsylvania, 17241. She is single. The father of the child is Gary L. "ess, currently residing at 1812 Jumper Street, Cayce, South Carolina, 29033. "e is single. 4. The relationship of Plaintiff to the child is that of natural mother. The Plaintiff currently resides with no other persons. 6. The relationship of Defendant to the children is that of natural father. Plaintiff does not know whether Defendant currently resides with any other persons. 6. Plaintiff has not participated as a party or witness, or in another capacity, in other litigation concerning the custody of the child in this or another court. Plaintiff does not have information of a custody proceeding concerning the child pending in a court of this Commonwealth. Plaintiff is not aware of a person not a party to the proceedings who hus physical custody of the child and claims to have custody or visitation rights with respect to the child. 7. The best interest and permanent welfare of the child will be served by granting the relief request becausel a.
