For sewer services rendered from 07/01/01 to 08/31/02. Said services were rendered
pursuant to ordinance. Attached hereto is a statement of the kind end character of the
services so rendered and the price charged therefor, said statement being a true copy of
the owner'S account with Hampden Township which sum of $527.92 was duly assessed
against the hereinabove-named owner and the premises on 07/01/01 and for which sum, with
interest, penalties and costs, a lien is claimed against the above-described premises in
accordance with the Act of May 16, 1923, P.L. ;>07 (53 P.5.5action 7101, et seq.), as
amended and supplemented, and the Act of May 2, 1945, P.L. 382 (53 P.5.5action 301,
et seq), as amended and supplemented.
