AND NOW, comes Dutchie Vashey, by and through her attorneys,
Reager, Adler, P.c., and avers the following:
1. Plaintiff is Dutchie Vashey, an adult individual
currently residing at 18 Palmer Drive, Camp Hill, Cumberland
County, Pennsylvania, 17011.
2. Defendant is RiChard A. Vazquez, an adult individual
currently residing at 376 Park Avenue, Perth Amboy, New Jersey,
08861.
3. Plaintiff seeks custody of the fOllowing child:

4. The child was born out of wedlock.
5. The child is presently in the custody of his mother,
Dutchie Vashey, who currently resides at 18 Palmer Drive, camp
Hill, Pennsylvania, 17011.
6. Since birth, the child has resided with the following
persons and at the following addresses:

7. The mother of the child is Dutchie Vashey who currently
resides at 18 Palmer Drive, camp Hill, Pennsylvania, 17011. She is
single.
8. The father of the child is Richard A. Vazquez who
currently resides at 376 Park Avenue, Perth Amboy, New Jersey,
08861. He is single.

9. The relationship of the Plaintiff to the minor child is
that of natural mother. The Plaintiff currently resides with her
aunt.
10. The relationship of the Defendant to the minor child is
that of natural father. Defendant resides with his brothers.
11. Plaintiff has not participated as a party or witness, or
in another capacity, in other litigation concerning the custody of
the child in this or another court. Plaintiff has no information
of a custody proceeding concerning the children pending in a court
of this Commonwealth. Plaintiff does not know of a person not a
party to the proceeding who has physical custody of the children or
claims to have custody or visitation rights with respect to the
children.
12. The best interest and permanent welfare of the children
will be served by granting the relief requested for the following
reasons:
(a) the mother has had physical custody of the child
since August and it is in the best interest of the child to
maintain this continual contact with mother; and

conciliation while the Defendant did not, but that the Defendant was served with the
custody complaint and was aware of the time and date of the conciliation, it is hereby

1. The parties will share legal custody of their minor child, Richard
Troy Vazquez, d.o.b. 19 May 1994.
2. Mother will have primary physical custody of the minor child
subject to periods of partial custody and visitation with the Father as agreed
upon by the parties.
3. In the event that the Father believes that he is being denied proper
access to the child, he may petition the court for another conciliation so that
a more formal visitation and partial custody schedule mPj b~ut In place.

6. The Plaintiff's position on custody Is as follows: Plaintiff wanted to have
custody confirmed In her since she Indicated that the Father of the child has
threatened to take the child and keep the child in New Jersey where he currently
resides.
6. The Defendant's position on custody is as follows: The Defendant did
not appear. Plaintiff and her counsel indicated that the Defendant had contacted
them and knew about the conference. Plaintiff's counsel Indicated that she sent
the notice by certified mail and is awaiting receipt of the return receipt card. She
will file thst return receipt card shortly.
7. Need for separate counsel to represent child: None requested and the
Conciliator does not believe any is necessary.
8. Need for independent psychological evaluation or counseling: None
requested and the Conciliator does not believe It is necessary.
