AND NOW comes James C. McCanna by and through Frey and Tiley, allomeys for
Plaintiff, and makes Ihe following slatemenJ:
1. Plaintiff is James C. McCanna, an adult individual, who currently resides al 110
Meals Drive. Carlisle. Cumberland Counly. Pennsylvania.
2. Defendanl is Jo Anne McCanna. an adulJ individual, who currenlly resides at 535
Soulh Middlesex Road. Carlisle. Pennsylvania.
3. Plainliff and Defendanl have been bona fide residenls in Ihe CommonwealJh of
Pennsylvania for allea.~1 six (6) months immediately previous 10 Ihe filing of Ihis Complaint
4. Plaintiff and Defendant were married on September 5, 1992 in Winchester.
Virginia.
5. There have been no prior actions of divorce or for annulmenl belween the parties.

OF INTENTION TO REQUEST ENTRY OF DIVORCE DECREE,
AND WAIVER OF MARRIAGE COUNSELING
J. A Complaint in divorce under Section 3301(c) oflhe Divorce Code was filed on February
2,1996.
2. The marriage of Plaintiff and Defendant is irretrievably broken and ninety days have
elapsed from Ihe date of filing und service of the Complaint.
3. I consenlto Ihe entry of a finw decree of divorce wilhout noticc.
4. I understand Ihatl may lose rights conceming alimony. division of property, lawyer's
fees, or expenses if i do nOI claim Ihem before a divorce is grunted.
5. I undersland Ihall will nOI be divorced until a divorce decree is enlered by Ihe Court und
thai a copy of Ihe decree will be sent 10 me immediately after it is filed wilh the Prothonotary.
