resideH at 7933 Ryd,!r Lane, Hununelstown, Pennsylvania. He is
married.
4. The relaticlllship of ttw Plaintiff to thp. child is that
of mother. The Plaintiff currently resides with the child and
with her father, Daniel Ickes/ har sister, Kristie Ickes/ and her
friends Barb Wilson and Christie Marrow.
5. The relationship of the Defendant to the child is that
of father. He currently resides with his parents/ Gerald and
Elizabeth Foreman.
6. The Plaintiff has not participated as a party or
witness, or in another capacity, in other litigation concerning
the custody of the child in this or in another court.
The Plaintiff has no information of a custody proceeding
concerning the custody of the child in this or in another court.
Plaintiff does not know of a person not a party to these
proceedings who has physical custody of the child or who claims
to have cuntody or visitation rights with respect to the child.

a. Every other weekend from Saturday morning at 10100
a.m. until Sunday >lvening at 6:00 p.m.
b. Every Wednesday avaning from 6100 p.m. until 8100
p,m.
c. Every other holiday from 10100 a.m. until 8:00 p.m.
The holidays shall include Easter, Ml!morial Day/ Independl.!nce
Day/ Labor Day, and Thanksgiving Day. The holiday schedule shall
always take preference over the regular weekend custody schedule.
d. At such other times as the parties mutually agree.
4. The Christmas holiday will be divided into two segments:
Segment A shall commence on Christmas EVil! at 10100 a.m. and shall
terminate on Christmas Day at 10:00 a.m. Segment B shall
conunence on Christmas Day at 10:00 a.m. and shall terminate at
8:00 p.m. on Christmas Day. Father shall have ehe child during
Segment A in 1996,< and Mother will have Segment B. The parties
will alternate Segments in subsequent years.
5. The child shall spend Father's Day with Father and
Mother's Day with Mother regardless of the regular weekend
custody schedule.
