1. Plaintiff, THE PRUDENTIAL HOME MORTGAGE COMPANY, INC., is a
corporation organized under the laws of the State of New Jersey with
an office at 3200 Robbins Road, Springfield, Illinois 62794.
2. Defendant, THOMAS A. JOHNSTON, is an adult individual whose
last known address is 616 Erford Road, Camp Hill, PA 17011.
3. On or about September 11, 1992, the said Defendant executed and
delivered a Mortgage Note in the sum of $66,300.00 payable to GMAC
Mor'g.ge Corpor"ion of PA, which NO'e i. ""ched here,o 'nd m'rked

4. Contemporaneously with and at the time of the execution of the
aforesaid Mortgage Note, in order to secUre payment of the same,
Defendant made, executed and delivered to original Mortgagee, a
certain real estate Mortgage which is recorded in the Recorder of

The mortgage was SUbseqUently assigned to The Prudential Home

incorpOrated herein by reference.

1. The plaintiff is carol J. canoy, paternal grandmother of
the subject minor child who currently resides at 142 Bosler
Avenue, Lemoyne, CumbeI'land county, Pennsylvania 17043.
2. The Defendants are ~ichael A. Canoy who currently resides
at 142 Bosler Avenue, Lemoyne, Cumberland County, pennsylvania
and Christina R. Hoffer who$e last known address is 51 Wyoming
Avenue, Lot 14, Middletown, Dauphin county, pennsylvania 17057.
3. Plaintiff seeks primary legal and physical custody of the
following child: Kaitlyn Christine Canoy, born October 23, 1992,
who currently resides at 142 Bosler Avenue, Lemoyne, Cumberland

child. A copy of the most recent Order is attached hereto.

emotional and spiritual wellbeing of the child.
