Canoy who currently resides at 142 Bosler Avenue, Lemoyne, Cumberland County, Pennsylvania and Christina R. Hoffer whose last known address is 51 wyoming Avenue, Lot 14, Middletown, Dauphin County, Pennsylvania 17057. 3. Plaintiff seeks primary legal and physical custody of the fOllowing child: Kaitlyn Christine Canoy, born October 23, 1992, who currently resides at 142 Bosler Avenue, Lemoyne, Cumberland County, Pennsylvania. 4. The child was born out of wedlock. 5. The child is presently in the custody of Carol J. Canoy, the paternal grandmother, who resides at 142 Bosler Avenue, Lemoyne, Cumberland County, Pennsylvania. 6. Plaintiff has no information of a custody proceeding concerning the child pending in a court of this Commonwealth except as referred to above. 14.
