The Secretary of the Commonwealth is directed to note such revocation and cancellation upon the
Volunteer Police Officer docket.
The Recorder of Deeds of .................... Gtun~~rland. _ County is authorized and requested
to fiie this Revocation and Cancellation, attached to said commission on file in his offiice, and to return
to the Secretary of the Commonwealth, Harrisburg, Pennsylvania, the commission card if such re-
mains attached to the commission.

1. TO THE FORMER VOLUNTEER POLTCE OFFICER WHOSE COMMISSION IS
I-iEREBY REVOKED AND CANCELLED.
Inasmuch as all your powers as a Volunteer Police Officer have been revoked
and cancelled by the Governor, you must surrender immediately your commission
card (Identification Card) by forwarding it to-
Governor's Office, Harrisburg, Pa.
2. TO THE POLITICAL SUBDIVISION OR INDUSTRY WHICH SPONSORED THE
VOLUNTEER POLICE OFFICER'S APPLICATION FOR APPOINTMENT.
Your records should indicate that the Commission of the person named on the
reverse of this form has been cancelled and rev~~ked on the date shown. (This
copy of revocation and cancellation is for your file)
