Volunteer Police Officer docket.

to file this Revocation and Cancellation, attached to said commission on file in his office, and to return
to the Secretary of the Commonwealth, Harrisburg, Pennsylvania, the commission card if such re-
mains attached to the commission.

Inasmuch as all your powers as a Volunteer Police Officer have been revoked
and cancelled by the Governor, you must surrender immediately your commission
card (Identification Card) by forwarding it to-
Governor's Office, Harrisburg, Pa.
2. TO THE POLITICAL SUBDIVISION OR INDUSTRY tiVHICH SPONSORED THE
VOLUNTEER POLICE OFFICER'S APPLICATION FOR APPOINTMENT.
Your records should indicate that the Commission of the person named on the
reverse of this form has been cancelled and rev>>ked on the date shown. (This
copy of revocation and cancellation is for your file)
TO THE POLICE DEPARTMENT, POLTCE COMP,2ISSION, OR OTHER OFFICIAL
PERSON OR PERSONS DESIGNATED BY THE GOVEF~NOR ON THE CAN-
