Inasmuch as all your powers as a Volunteer Police Officer have been revoked
and cancelled by the Governor, you must surrender immediately your coinrnission
card (Identification Card) by forwarding it to-
Governor's Office, Harrisburg, Pa.
2. TO THE POLITICAL SUBDIVISION OR INDUSTRY WHICH SPONSORED THE
VOLUNTEEF~ POLICE OFFICER'S APPLICATION FOR APPOINTMENT.
Your records should indicate that the Commission of the person named on the
reverse of this form has been cancelled and revoked on the date shown. (This
