Inasmuch as all your powers as a Volunteer Police Officer have been revoked and cancelled by the Governor, you must surrender immediately your commission card (Identification Card) by forwarding it to— Governor's Office, Harrisburg, Pa. 2. TO THE POLITICAL SUBDIVISION OR INDUSTRY WHICH SPONSORED THE VOLUNTEER POLICE OFFICER'S APPLICATION FOR APPOINTMENT. Your records should indicate that the Commission of the person named on the reverse of this form has been cancelled and revoked on the date shown. (This copy of revocation and cancellation is for your file) 3.
