TO THE POLITICAL SUBDIVISION OR INDUSTRY WHICH SPONSORED THE VOLUNTEER POLICE OFFICER'S APPLICATION FOR APPOINTMENT. Your records should indicate that the Commission of the person named on the reverse of this form has been cancelled and revoked on the date shown.
